Application Form. Online application form, printed and signed.

Passport-type photo. 2 passport-size photographs (5x7cm), with your name signed on the back, and your application number written clearly on the back of the photo.

Previous passport. Photocopies of any previous passports you have in your possession.

Personal Letter. Letter that follows the following format. Type or write a letter explaining why you want to come to Ireland. Sign and date the letter and submit it with your documents. Your letter must include your full name and postal address, and the:
- Reason you want to come to Ireland
- Dates you plan to arrive and leave
Your letter must also include a commitment from you that you will: - Obey the conditions of your visa in full
- Not rely on public services (eg public hospitals) or become a burden on the State
- Leave Ireland before your immigration permission expires.

Vacation/holiday plan. Typed or written vacation/holiday plan, separate from your Personal Letter.

Bank Statement. Copy of a recent bank statement, showing summaries of the amount of money withdrawn and deposited in the last SIX MONTHS.

Medical insurance. Proof of valid medical insurance that covers your whole trip to Ireland.

Employment Letter. Employment Letter or confirmation of enrollment with your school, stating you are granted a leave of absence and will be returning to work/school after your trip.

Family Letter. If you have family in Canada, write or type a letter explaining who they are, how they're related to you, and that you intend to return to them.
If you are married, you must submit your original Marriage Certificate.
If you have children, you must submit their original Birth Certificates.
If you have ever had a visa refused by ANY country, write or type a letter explaining which country refused your visa, and why you believe it happened.
