Request letter. Written letter addressed to the Consular Section of the Embassy of Bolivia, requesting Certification Travel Permit for children under 18 years. Address, telephone and email must be included.
Travel Permit. Completed in Spanish. This form is available here.
Proof of identity. Photocopy of passport and / or identity card and birth certificate of the child.
Cost of certification. $9.00 USD paid by money order or certified check made out to Embassy of Bolivia.

Bolivia tourist visa is required for citizens of Eritrea.
