Passport-type photograph: 1. Photo size MUST 2 inches x 2 inches. Please ensure that the background of the photograph is plain white. The photograph should not be older than 3 months and ensure your full name and the print date are written and stamped respectively at the back.
Signed Application Form. Print and sign the application form once it is e-mailed to you by a VisaHQ agent.
Yellow Fever Vaccination.

Hotel Reservations. Copy of confirmed hotel reservations.
Hotel Reservations. A signed letter of confirmed hotel reservation for the duration of stay in Nigeria with photocopy of employee’s identification from the hotel.
Personal Letter. A personal letter from the applicant addressed to the Embassy of Nigeria, explaining the purpose of the trip, dates of travel, cities to be visited, and places of accommodations.
Letter of Introduction. A short letter from the applicant's employer introducing the applicant to the High Commission of Nigeria in Canada, briefly detailing who the applicant is, where they reside, where they work and for long they have been employed. The letter should also include the reason for traveling, the dates in which you will be in Nigeria and where you intend you stay during your trip. The letter should be signed, dated and addressed to the High Commission of Nigeria in Canada.
Evidence of Sufficient Funds. Photocopy of receipts for the purchase of U.S. funds from the bank considered adequate to cover the cost of the trip and not statement of account.
